Person Centered Planning Facilitation


Person Centered Planning assists individuals in creating a personalized image of a desirable future. The development of a plan suggests a process that can organize and guide community change in alliance with people with disabilities thus bridging the gap from both sides. Essential components of PCP’s are that they: are person directed and person centered, promote capacity building, network building, community building and offer the dignity of choice and risk. As facilitators we seek to blend the use of natural, community, family, and paid supports. It is understood that as facilitators of the PCP process inquiry and focus on the person and/or family’s dreams and desires is the most important component. The Arc’s role is one of facilitating and guiding not advocating. 

Activities of Person Centered Planning (PCP) Facilitation

Pre-Planning Meeting: Based on consumer’s preference the pre-planning can occur in a meeting or by phone. The Arc believes that in most cases a meeting may be the most appropriate option. 

PCP Gathering: This is the actual PCP meeting of identified people the person and/or family wants at the event. Dreams, desires, and supports will be discussed to plan for the upcoming year. 

Follow-up: This includes any necessary follow-up activities by phone, etc. In some cases, it may be necessary to meet more than one time.
